IPAP TA Memo Request
Thank you for your interest in the UNMC Interservice Physician Assistant Program (IPAP)!
UNMC offers an unofficial prerequisite coursework evaluation for individuals interested in determining whether they have completed the admission prerequisite requirements for IPAP. If needed, we can also provide a Tuition Assistance (TA) Memo for submission to an advisor or education center. Please note that this evaluation is not required to apply to IPAP.
Before submitting this request, please carefully review the IPAP prerequisite requirements for your branch of service. In many cases, applicants can determine whether their coursework meets the published requirements without the need for an individual evaluation. Because of the volume of requests received, we encourage you to submit this form only if you are uncertain whether one or more courses satisfy the prerequisite requirements or if you require a Tuition Assistance (TA) Memo.
Please complete the form below and list all colleges, universities, and programs you have attended. You will also be asked to upload documentation from each institution showing all completed coursework and grades.
Our staff will review your records and complete an unofficial prerequisite coursework evaluation. Based on the option you select, we will either:
- Provide a determination of whether your prerequisite coursework requirements have been met, or
- Issue a Tuition Assistance (TA) Memo documenting your coursework status.
Please allow 3–4 weeks from the date of submission for your evaluation to be completed.
